Pt. B.D. Sharma University of Health Sciences, Rohtak invites applications for admission to Bachelor of Physiotherapy (BPT) & B.Sc. Optometry course 2011 in College of Physiotherapy, PGIMS, Rohtak, Other BPT College affiliated to Pt. BD Sharma University of Health Sciences, Rohtak and B.Sc Optometry course at Regional Institute of Ophthalmology, PGIMS, Rohtak as per the following schedule:

Last date for receipt of application form: 13.06.2011 up to 5.00 pm

Date of Entrance Test at Rohtak: 10.07.2011 (Sunday)

Paper: 02.00 PM to 5.00 PM

Eligibility

The candidate should attain the age of 17 years of more on 31.12.2011

The candidate must have passed following examinations

Senior Secondary Certificate Examination (10+2) of Board of School Education Haryana or equivalent examination with a minimum of 50% marks in the aggregate in the subjects of Physics, Chemistry, Biology and English taken together and pass the qualifying examination.

In case of SC/ST candidates the marks obtained should be read as 40% in qualifying examinations.

For B.Sc. Optometry

In service candidates shall also be eligible as in-service candidates provided he/she fulfills above qualifications for eligibility has passed the course of Ophthalmic Assistant/Optometry/Orthoptics (minimum 2 years duration) from any recognized Board/Council/University and thereafter is in regular employment with Haryana Health/Medical Education and research department and possesses a minimum experience of three years or more as Optometrist/Ophthalmic Assistant in Haryana Health/Medical  Education and Research Department.

The prospectus containing eligibility conditions, domicile criteria, application form and OMR form can be obtained from the office of the Controller of Finance, Pt. B.D. Sharma University of Health Sciences, Rohtak & HDFC Bank, Near D-Park, Model Town, Rohtak on cash payment of Rs. 500/-(Rs. 125/- for SC/BC of Haryana) w.e.f. 20-05-2011. 

Request for prospectus through Registered Post must be received in the office of the Controller of Finance, on or before 07-06-2011 by sending a Demand Draft of Rs. 550/- (Rs. 175/- for SC/BC of Haryana) drawn in favour of “Controller of Finance, Pt. B.D. Sharma University of Health Sciences, Rohtak Payable at State Bank of India, Pt. B.D. Sharma PGIMS Rohtak (Code 4735) failing which the request will not be entertained.

The Application form complete in all respects must reach the Controller of Examinations, Pt. B.D. Sharma University of Health Sciences, Rohtak by 30.06.2011 upto 05.00 PM.  The application received last shall be rejected.
