M.Ch (Neuro-Surgery); M.Ch (Urology); DM (Nephrology): Sunday, 05th June, 2011

Eligibility criteria, centres for test, last date of submission of forms, test fees and other details are available on the university website

Application form can be obtained from the university office on payment of Rs 500/- in cash or demand draft (for super-specialty programmes Rs 1000/-);or through a requisition letter with name of course, opted for, detailed correspondence address with contact number and e-mail address, along with a demand draft of Rs 600/- (for super-specialty programmes Rs 1100/-) drawn on any nationalized bank, in favour of registrar, Dr DY Patil Vidyapeeth payable at Pune; or by downloading from the university website (the downloaded form shall be submitted along with a demand draft covering forms fee + test fee + postage Rs 100/-)

Fees should not be sent by money order

The university shall not be responsible for any postal delay and loss

Late fee for all courses shall be Rs 500/-

Information brochure for MBBS/BDS can also be obtained from selected branches of Andhra bank all over India after 25th January, 2011

The list of Andhra bank branches is available on the university website; information brochures for other courses will be available in the university office and on the university website
