Application form and prospectus can be held on payment of Rs 1500/- at the counter from Sri Guru Ram Das Institute of Medical Sciences and Research, Mehta Road, Sri Amritsar-143501; Sri Guru Ram Das Institute of Dental Sciences and Research, GT Road, Sri Amritsar-143001; Sri Guru Ram Das Charitable Hospital, Near Gurdwara Shaheedan, Sri Amritsar; SGPC Sub-Office, Kothi No-20, Sector-5, Chandigarh; or by sending letter through registered post/speed post only, and not by courier to Sri Guru Ram Das Institute of Medical Sciences and Research, Mehta Road, Sri Amritsar-143501 along with a demand draft for Rs 1600/- in favour of Registrar Examination, AUSMMDI, payable at Amritsar
